  • Aviva Recovery accepts referrals from CCS in Eau Claire County at this time. Stay tuned, as we aim to expand to additional counties in Western Wisconsin in the near future!

    Aviva also accepts private pay. Private pay services are provided at a rate of $60 per hour. For those who are facing financial hardship, we offer a sliding fee scale based on an honor system that allows for payment starting at $20 per hour. Please reach out to us at amy@avivarecovery.com or 715-204-9848 to request assistance. No income documentation is required.

  • In order to best meet the needs of the individuals requesting services, Aviva offers a sliding fee scale based on an honor system. While the private pay rate is set at $60 per hour, Aviva recognizes that this is not affordable to everyone. We never want the ability to pay to be a barrier to receiving services. As such, we offer the following honor system, in an attempt to meet the needs of our community.

    With an honor system, no proof of income or paperwork is ever required. Just choose the rate that feels doable for you right now:

    $20–25 → If money is really tight right now or you're going through a hard season financially

    $30–40 → If things are stable but you’re still needing to be mindful of your budget

    $45–55 → If you’re doing okay overall, but would appreciate some flexibility

    $60 → If you’re able to comfortably pay the full rate

    You choose what you feel comfortable with. We trust that you know what fits your situation best.

  • CCS stands for Comprehensive Community Services, which is a voluntary Medicaid funded program designed for individuals coping with a mental health and/or substance use condition. CCS serves both adults and children.

    Each individual who participates in CCS works with their team to develop a recovery plan with distinct goals intended to help build the skills needed to improve health, promote wellness, and enhance the Participant’s overall quality of life. Eligibility is determined by a Functional Screen, and the Participant must be enrolled in Medicaid to participate.

  • Meetings can occur where you feel best, whether that’s in your home or out in the community. What each session looks like will depend on your goals. We can meet up at a coffee shop, go for a walk, or work on goals from your kitchen table.

    At Aviva, we believe it’s important to connect and catch up each time we meet, but we also want to honor your goals and help you make progress toward achieving them. It’s important to us to be flexible and to get creative in how we work together. We commit to always keeping the wishes and needs of the individual front and center.
